3 Easy Steps
We recognize that this process may seem somewhat confusing and overwhelming. To help we have created the following guide that takes you step by step through the ordering process.
1. Account set ups: For yourself and one for your physician
Before searching for donors on the donor search, you will need to set up an online account. This will allow you to view donor information.
For those ordering semen vials by phone for the first time, you must read the Terms of Use and sign electronically or print, complete the final page of this document and return it at least one business day prior to shipment of any vials. This important document explains what to expect from our donor program and what we expect from you when purchasing and utilizing our services. If you are ordering semen vials online for the first time, the Terms of Use is available online for you to read and submit (by clicking an online button) prior to completing your purchase.
Fairfax Cryobank requires that all parties are working with a Licensed Physician/Medical Professional. Our Client Services department can verify if your physician already has established an account by calling 800-338-8407. If an account has not already been established, a physician account set up form can be faxed directly to your physician's office by providing a fax number to one of our Client Services Specialists. If you do not already have a licensed physician, we will be able to provide you a list of physicians in your area who specialize in reproductive medicine.
2. Choosing Your Donor Match
The Fairfax Cryobank Donor Search and the Client Services Specialists are useful tools to assist you in making your important donor selection. Our interactive donor search which is updated daily allows you to search by several specific criteria that are most important to you. Please be aware that if you choose an ID Option donor, you will be required to complete and return a signed ID Option Patient Agreement. This form is donor specific and will need to be mailed, faxed or scanned and emailed at least one business day before you place your order.
Additional information is available on our donors. Items available on donors include childhood photos, Lifetime photos, staff impressions, silhouettes, donor essays, personality testing, audio interviews, medical profiles, and personal profiles. If you prefer, you can choose to have items mailed to you for an additional fee. We also offer photo matching and donor consultation services. Meet our newest donors here. Our client services team can answer any inquiries you might have, provide donor availability, and direct you through the process.
Due to high demand for certain donors, we recommend having several donors selected (at least three) in case your preferred donor sells out. You also have the ability to select a donor well in advance of your procedure and purchase specimens now and have them stored until you need them. 3. Place the Order
Either you or your health-care provider may place an order. Before placing an order, please check availability online or by calling Client Services. We recommend placing your first order over the phone. This allows us to work together placing the order and helps prepare you for subsequent orders. You may do so by contacting Client Services at 800-338-8407. We also provide online ordering for donor sperm and additional information through our Donor Search.
At the time of your order you will need to have the following:

Patient name, address, and phone number

Medical Professional's name and shipping address

Donor number

Number of units and preparation type (IVF, ICI or pre-washed IUI)

Date the units need to arrive at the provider's office

Payment information
Customer Service takes orders 7am to 6pm Monday - Friday, Eastern Standard Time (EST).
Please be aware that the cut off time for same day Fed Ex order is 4:00PM EST; and same day local courier cut off time is 11:00AM EST.
Fairfax Cryobank accepts all major credit cards, money orders, and cashier checks. See more information on financing options here. Personal checks are not accepted. Fairfax Cryobank must receive any money order or cashier check before the shipping date. We do not work with or arrange payment with insurance companies, although we can provide you with copies of paid invoices to submit to insurance upon request.
Fairfax Cryobank suggests calling your health care provider with the delivery information once you have placed the order.
